Transformation Collaborative

Our mission is to empower health centers in transforming their practices to
achieve high quality patient centric care.

A core focus of the Iowa Primary Care Association’s work is to support quality primary care services in Iowa. Performance improvement (PI) activities, information and resources are provided to member health centers and partners to help address clinical, management, operational, finance and governance functions that can be impacted by continuous quality improvement/assurance processes. Our quality program’s activities are designed to promote the sharing of ‘best practices’ and improve communications between professionals across the state. Clinical performance outcomes, financial operations benchmarks and performance outcome data standards for measuring health center performance are all areas of growth. We are working to help strengthen the quality improvement infrastructure of each health center and align the federal funding and reporting requirements to emerging areas of emphasis such as patient centered medical home and meaningful use. Additionally, we work to position health centers to make sure the entire population is healthy and will strive to meet the Healthy People 2020 Goals.
